1. Agreement to These Terms

By using gentlegrandprairiedentist.com, you agree to these Terms of Use. If you do not agree, please do not use the site.

This website is owned and run by Grand Prairie Family Dental, the general and family dental practice of Dr. Behrooz Khademazad, DDS, Texas License #15515, located at 2475 W Pioneer Pkwy, Grand Prairie, TX 75051. In these Terms, “we,” “us,” and “our” mean Grand Prairie Family Dental. “You” means anyone who visits or uses the site.

These Terms cover every page, form, quiz, cost estimator, guide, and download on the site. Our Privacy Policy explains how we handle your information and is part of these Terms.

The site is meant for people 18 and older. A parent or legal guardian should complete any form or request for a child.

2. Educational Information Only, Not Dental Advice

Everything on this site is general education. It is not a diagnosis, a treatment plan, or dental or medical advice for you. Articles, guides, videos, and FAQs explain common topics. They cannot replace an exam, X-rays, and a talk with a dentist who knows your health history.

Using the site does not make you a patient of Grand Prairie Family Dental. A dentist-patient relationship starts only after you are seen in our office and agree to care.

Do not delay or skip care because of something you read here. Always talk with a licensed dentist or physician about your own symptoms, medicines, and treatment choices. Results of any treatment vary from person to person.

Emergencies

If you have trouble breathing or swallowing, swelling that spreads to your eye or neck, a high fever with facial swelling, heavy bleeding that will not stop, or a jaw or face injury, call 911 or go to the nearest emergency room now. Do not use a website form for an emergency. For an urgent dental problem during office hours, call us at 972-988-0900.

3. Quizzes, Cost Estimators, and Self-Check Tools

Our site offers interactive tools, including the Smile Quiz, Sleep Apnea Quiz, Kids Sleep Apnea Quiz, TMJ Quiz, Dental Implant Quiz, Wisdom Teeth Removal Quiz, Invisalign Questionnaire, and the Dental Treatment Cost Estimator, Dental Implants Cost Estimator, and Invisalign Cost Estimator.

These tools help you think through your questions before a visit. Please keep these limits in mind:

  • A quiz result is not a diagnosis. It cannot confirm or rule out sleep apnea, TMJ disorder, gum disease, or any other condition.
  • A sleep apnea quiz is a screening aid only. Sleep apnea is diagnosed by a physician, often with a sleep study.
  • A cost estimate is a rough range, not a quote or a promise. Your real cost depends on your exam, X-rays, the treatment you choose, lab fees, and your insurance plan.
  • Answers you enter are only as accurate as the information you give.

We give you a written treatment plan with costs after your exam and before any treatment starts. That written plan, not an online tool, is what counts.

4. Appointment Requests and Contact Forms

Sending a request appointment form or contact form does not book an appointment. Your visit is confirmed only when our office calls, texts, or emails you to confirm a date and time.

When you use a form, you agree to give true and current information. Please send only what we need to reach you, such as your name, phone, email, and the reason for your visit. Do not send detailed health history, Social Security numbers, insurance member IDs, or payment card numbers through a website form. We will collect those safely at your visit or through our office.

We reply to messages during office hours. A form is not watched around the clock, so never use one for an urgent or emergency need.

5. Costs, Insurance, Financing, and Special Offers

Costs

Any cost or price range on this site is general information. Costs vary by case. Your fee is set only after an exam and is shown in your written treatment plan.

Insurance

Insurance benefits vary by plan, employer, and carrier. Mention of an insurance carrier, plan type, or benefit on this site is not a promise of coverage. We are happy to check your benefits, but your carrier makes the final coverage decision. You are responsible for any amount your plan does not pay.

Financing

We may share information about outside financing companies on our dental financing page. Those companies set their own terms, rates, and approval rules. Approval is not guaranteed. Any financing agreement is between you and the lender, not Grand Prairie Family Dental.

Special Offers and Partnerships

Offers on our special offers page, the teeth whitening special, and any partner program, such as the Arlington ISD partnership, follow these rules unless the offer says otherwise:

  • Offers apply to new patients only when stated.
  • One offer per patient. Offers cannot be combined with other discounts.
  • Offers have no cash value and cannot be transferred or exchanged.
  • Offers may not apply to patients with government-funded plans such as Medicaid or CHIP, where state or federal rules forbid it.
  • We may change or end an offer at any time. An offer is honored as long as it was shown on the site when you booked and you mention it at booking.

An offer does not mean free care, and it does not change the clinical need for any treatment.

6. Reviews, Testimonials, and Photos

Reviews and testimonials on our testimonials and reviews pages share real patients’ own experiences. They are not a promise you will have the same result. Every mouth is different, and results vary.

We post a patient’s name, review, photo, or before-and-after image only with that patient’s written permission. Before-and-after photos show real cases and are not edited to change the clinical result.

If you send us a review, comment, photo, or other content, you confirm it is true, it is yours to share, and it does not include anyone else’s private health information. You give us permission to show it on our site and our social media pages, with the name you approve. You may ask us to remove it at any time by calling 972-988-0900.

We do not pay for reviews and do not offer discounts in exchange for positive ones.

7. Privacy, Health Information, and Messages

Your Privacy

Our Privacy Policy explains what we collect on this site, such as form entries, quiz answers, and basic site analytics, and how we use it.

Health Information

Once you become a patient, your dental records are protected health information. We handle them under the federal HIPAA Privacy Rule and the Texas Medical Records Privacy Act (Texas Health and Safety Code, Chapter 181). Our Notice of Privacy Practices, given at your first visit, explains your rights. Ask our front desk for a copy anytime.

Email and website forms are not fully secure. Please keep detailed health details out of them and share those by phone or in person.

Calls, Texts, and Emails

When you give us your phone number or email on this site, you agree we may contact you about your request, appointments, reminders, and your care. Message and data rates may apply. We send marketing texts only with your separate consent.

To stop texts, reply STOP. To stop marketing emails, use the unsubscribe link in any email. You will still get messages about your appointments and care unless you ask us to stop.

8. Site Content, Proper Use, and Outside Links

Who Owns the Content

The text, articles, guides, quizzes, photos, videos, graphics, and logo on this site belong to Grand Prairie Family Dental or are used with permission. You may view, print, and share pages for your own personal, non-commercial use. You may not copy, sell, republish, or change our content without our written permission.

You may link to any page on our site, as long as the link does not suggest we endorse you or your business.

Proper Use

When you use this site, you agree not to:

  • Send false information or pretend to be someone else
